You will be an executive in our large integrated healthcare organization, sharing your passion for whole person care as a Primary Care Provider, providing care in English and Spanish, and overseeing primary care teams leveraging your background in primary care and integrated healthcare.You will support our team-based primary care model that includes case management, nurses, and, with our recent merger, expanding our integration in whole person care with behavioral health professionals. Your passion for quality outcomes within the population health framework will guide your assessment from an evidence-based diagnosis perspective.At CHFW, you will manage value-based payments (VBP) and the VBP system, ensuring insurance payers receive our outcomes based on quality and deliverables. You will understand the requirements and know what is needed to achieve the required outcomes.In our collaborative environment, you will work with other executives and the senior leadership team to maximize outcomes for our patients. You will provide our Board of Directors with risk reporting through written reports and in-person presentations. • MD/DO, NP, or PA with board certification or appropriate training and/or experience in family medicine or other primary care specialties.
• Valid license/DEA in the jurisdiction of the clinic.
• Minimum of five years' progressive experience in medical management and leadership, preferably in a community health center.
