    • BLS Certification - AHA or ARC Only, Required for Onboarding
    • PALS Certification - AHA Only, Required for Onboarding
    • Nursys - Nursing verification, within 30 days of start
    • License/Certificate Verification - Primary Source Verification within 30 days of start, updated upon expiration
    • Skills Checklist - Unit specific, within one year prior to start date; renewed annually
    • Competency Exam - Passing score of 80% within year of start
    • Core Competencies - HIPAA, Patient Bill of Rights, Safety Orientation, etc., renewed annually
    • CPI - As required per specialty/Unit & Facility
    • MMR Immunizations - Proof required, positive titer or Declination accepted
    • Fit Test - Completeness and type specification
    • Hepatitis B - Proof of immunization or Declination accepted
    • Influenza Vaccine - Proof required, Medical/Religious exemption with signature
    • TB Test - Proof of negative Blood test or Skin test
    • Health Statement - Signed and dated within 1 year of start
    • Varicella - Proof of immunization or Declination accepted
