  • Perioperative Educator is responsible for the successful onboarding of new team members, as well as ensuring annual and ongoing education is conducted for all team members. Assist with orientation calendars and monitoring progress; regular rounding with new team members and preceptors to track progress. Assist with performing quality audits and creating education resources for gaps in practice.

    • Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870+. Must be completed within 1 year prior to start. Updated annually.

    • Proof of vaccine required from October - March. Only Medical/Religious exemptions will be accepted with a physician's signature(Medical) or written signed statement from traveler(Religious). Medical exemption request must be completed on the FMOLHS Medical Exemption Flu Form, please see Document Library.
    • Proof of negative Blood test or Skin test as described below. - QuantiFERON Blood Test within one year prior to start OR 2-Step TB Skin Test 1 within 30 days and the other within 1 year - If positive TB record; Must provide MD note stating traveler is free of active TB or provide proof of treatment for LTBI within 30 days of start.
    • Statement signed and dated within 1 year of start stating traveler is able to work without restrictions.
    • Proof of 2 immunizations, positive titer or Declination accepted.
    • Proof of vaccination within past 10 years or Declination accepted.
    • See document library. Required within 6 months of start. If POSITIVE documentation of evaluation from the Department of Health required. If treatment was prescribed, proof of completion required.
    • Background checks must be within 30 days prior to start. 7-year County of locations lived/worked in/in school, Nationwide Criminal, SSN Trace, and National Sex Offender, OIG, EPLS, OFAC, Education and Employment Verification, and Driving Record. Prefer all checks uploaded together.
