    • --- Unit specific to the contractor's scope of practice, within one year prior to start and renewed annually
      
    • --- Copies of exams needed, within year of start with passing score of 80% or higher, as per Franciscan Competencies Requirement
      
    • --- Annual compliance with Franciscan Competencies Requirement
      
    • --- Proof of 2 immunizations, positive titer, or declination accepted
      
    • --- Must be one of the 3 mask fit types (N95- 3M 1860, 1860s, or 1870+) and must be completed within 1 year prior to start, updated annually

    • --- Proof of vaccine required from October - March, with medical or religious exemptions accepted
      
    • --- Proof of negative Blood test or Skin test, as described below
